Understanding that you cannot use one statement to count as two elements.
Ex: "pain since last Tuesday,” cannot count as both timing and duration (HPI) and, “no back pain,” can’t count both musculoskeletal and neurological (ROS.
But, what about CC "cough feels better"; Although I agree CC s/b "follow up for cough" this is what the provider wrote.
There was no HPI and wanting to get some sort of credit (not mentioning what they actually submitted )
Can I break up CC ("feels better" - severity HPI) or, is that also considered "double dipping"...AKA coding folk
